How to access MRI services
To schedule an examination we require a referral form signed and completed by your physician. You may download and print this form from here.
The form can be sent to Welcome Back MRI and Pain Management Centre by fax at 250-828-1493 or email info (at) welcomebackcentre.com
Once we receive your requisition, a Welcome Back representative will call you to schedule your appointment.
If you are unable to attend your appointment, please call Welcome Back Centre at 250-828-6740 (local in Kamloops and area) or 1-866-759-2674 (Toll - Free). If you do not cancel your appointment within 24 hours of your scheduled time, a fee may apply.
